ПУБЛІЧНЕ УПРАВЛІННЯ ЕКСТРЕНОЮ МЕДИЧНОЮ ДОПОМОГОЮ В КРИЗОВИХ СИТУАЦІЯХ: ФОРМУВАННЯ СИСТЕМИ РЕАГУВАННЯ
Анотація
Розглянуто особливості публічного управління наданням екстреної медичної допомоги в кризових ситуаціях. Дано опис трьох можливих ситуацій, у разі настання яких має надаватися медична допомога — це звичайні, непередбачені та надзвичайні. Зазначається, що останні, як правило, супроводжуються браком необхідних для їх подолання ресурсів і мають ознаки кризової ситуації. Сформовано загальне бачення системи реагування на кризові ситуації, розкриваються складові, роль та функції кожної із п'яти виділених компонент цієї системи, підкреслюється їх взаємозалежність. Виокремлено соціально-економічні, нормативно-правові та організаційні механізми публічного управління, які мають бути задіяні при плануванні заходів з надання ЕМД у кожній із зазначених ситуацій. Запропоновано структурно-логічну схему впровадження системи реагування на зміни ситуації. Зроблено висновок щодо важливості планування та розробки можливих сценаріїв розвитку кризової ситуації на основі налагодження чіткої інформаційної взаємодії органів публічної влади з усіма службами екстреного реагування та створення інтелектуальної мережі, побудованої на обміні цифровими даними в реальному часі, яка використовуватиметься для виявлення та запобігання кризовим ситуаціям пов'язаним, зокрема, з несвоєчасним наданням медичної допомоги, відсутністю необхідних ресурсів.

PUBLIC MANAGEMENT OF EMERGENCY MEDICAL SERVICES IN CRISIS SITUATIONS: FORMATION OF A REACTING SYSTEM
Summary
Features of public administration of emergency medical care in crises are considered. There is a description of three possible situations in which medical care should be provided — these are normal, unforeseen and emergency. It is noted that the latter are usually accompanied by a lack of resources needed to overcome them and have signs of a crisis. The definition of a crisis situation is given. The distribution of necessary resources (human, material) and standards according to which medical care should be provided is determined for each of these situations. The general vision of the crisis response system is formed, the components, role and functions of each of the five selected components of this system are revealed, and their interdependence is emphasized. Socio-economic, normative, law, organizational and information mechanisms of public administration are singled out, which should be involved in planning measures to provide EMS in each of these situations. The structural-logical scheme of introduction of the system of reaction to changes of a situation is offered. It is concluded that the importance of planning and developing possible scenarios for the development of a crisis based on the establishment of clear information interaction of public authorities with all emergency services. In this case is important to create an intelligent network based on real-time digital data exchange, which will be used to detect and prevent crises related to, in particular, the untimely provision of medical care, and lack of necessary resources. This network will used to automate notifications of possible emergencies, natural disasters, and decision-making in crises. Emphasis is placed on the fact that the use of digital technologies and network communications in crisis situations creates the conditions for efficient use and management of all available resources. In addition, it contributes to better control, management and monitoring of emergency services, the effectiveness of decisions taken by public authorities in the field of life safety, thereby reducing costs, increasing the efficiency and increasing the sustainability of the health care system. What is stated in this article serves as a methodological guide to further explorations in the direction of infrastructural transformation of EMS.
